Liability Release Form

In connection with (type student’s name) (hereinafter “Student”) participating in or attending classes at The Harid Conservatory, the undersigned, being the parent(s) or legal guardian(s) of the Student, do hereby, for ourselves, our heirs, assigns, executors and administrators, covenant not to sue, remise, release and forever discharge Harid Conservatory of Music, Inc., a Florida corporation not-for-profit, its agents, employees, independent contractors, officers, and trustees (hereinafter “Conservatory”) and further agree to indemnify and hold The Harid Conservatory harmless from and against any claim, demand, cause of action, suit, liability, damage, judgment, levy, or execution (hereinafter collectively “Claims”) including any attorneys’ fees and court costs incurred by the Conservatory in prosecuting or defending any such suit, action, claim, or counterclaim, arising out of the death or injury of the Student or the loss or destruction of any personal property of the Student while present at the Conservatory, at any residence facility provided by the Conservatory, or any other location in connection with the Student’s participation in any program, class, or activity offered by the Conservatory, whether or not caused, in whole or in part, by the Conservatory, its agents, employees, independent contractors, officers, trustees or representatives’ negligence, omissions or otherwise.

In further consideration of the Student participating or attending the classes at the Conservatory, the undersigned do hereby agree that we will not institute, prosecute, or in any manner whatsoever, aid in the institution or prosecution of any claim, demand, cause of action or suit against the Conservatory.

Further, the undersigned hereby authorizes the Conservatory to make arrangements for and obtain any type of emergency medical treatment whatsoever for the Student, which in the Conservatory’s sole opinion, is or may be necessary or appropriate for the Student. The undersigned hereby releases the Conservatory from and agrees to indemnify and hold the Conservatory harmless from and against any claims, damages, suits, costs, or expenses incurred relative to any emergency medical treatment provided to the Student or arranged for the Student’s benefit by the Conservatory.
